Better Choices, Better Health

An interactive 6-week workshop on self-management skills for chronic health conditions, diabetes, or cancer survivors.

Caregiver Counseling

Counseling caregivers to assist them in making decisions and problem-solving.

Case Management

Social work assistance to frail elderly; coordinates community and in-home services.

EnhanceFitness

An evidence-based, physical activity program designed to improve the overall functional fitness and well-being of older adults.

Fall Prevention and Wellness Program

In-home assessment that includes a short home visit by Agency on Elderly Affairs, Kauai Fire Department and American Medical response.

Grandparents Raising Grandchildren

Family caregivers (grandparents) have supportive programs and services that address their needs to enable them to continue giving care.

Homemaker

Helping frail older adults with housework, meal preparation and other limited in-home services. To qualify, someone must not be able to do one or more activities of daily living (ADLs) or instrumental activities of daily living (IADLs).

Home Service Registry

Referral of in-home workers who provide services for a fee.

Kauai Adult Day Health

Provide adult day care services in a supervised, protective, and congregate setting during the day.

Kupuna Care Transportation

Bus service available for registered seniors to and from program activities, shopping, clinic, recreation, and door-to-door services for frail elderly.

Legal Assistance

Legal education, counseling and referral in Medicaid, Medicare, landlord-tenant, creditor, family law and other legal matters.
Limited document preparation.

Nutrition Services

Hot lunches at various congregate meal sites and island-wide home-delivered meals to homebound seniors.

To qualify for a home delivered meal, an individual must be 60+ years old who is homebound and frail with two or more impairments and an unmet need, and meet at least one of the following:

  1. Lack the physical mobility and/or mental capacity to shop for food, prepare meals themselves and have no one to assist. 
  2. Lack adequate cooking facilities.
  3. OR is convalescing following discharge from hospital and does not have help to prepare meals.

Screening criteria will be provided to determine eligibility. Voluntary donation is accepted.

Respite

Provides caregiver relief by assisting in the supervision and care of frail elderly.

RSVP Volunteer Program

Meaningful volunteer opportunities for persons 55 years and older.

SHIP HAWAII - State Health Insurance Assistance Program

One-on-one Medicare counseling.

Senior Identification Card

Senior identification cards are available to seniors 60 years and older.  IDs are issued twice a month.  Appointments are recommended by calling 241-4470. A consumer registration form must be completed. A donation for the cost of the materials used is accepted.
